- 1. provision of medical services in the field of issuing certificates during participation in meetings of adjudicating teams in the District Disability Assessment Team in Poznań. The Ordering Party requires that the order be carried out by a team of doctors consisting of at least certifying doctors in the following specializations: a) pediatrics - 1 person;b) neurology - 1 person;c) orthopedics - 1 person;d) rheumatology 1 person;e) internal medicine 1 person;f) surgery 1 person;g) ophthalmology 1 person; 2. The certifying doctor is obliged to have a valid certificate issued in accordance with Regulation No. 23 of the Minister of Economy, Labor and Social Policy of July 15, 2003 on determining disability and the degree of disability (Journal of Laws of 2021, item 857, as amended) or committing to undergo training in accordance with the principles set out in 21(2) of this legal act; - the chairman of the adjudicating panel is: 1) a doctor with a specialist title or 2) a doctor with at least the first degree specialization, or3) a doctor undergoing specialization training in a specific field of medicine, who has completed the basic module appropriate for a given specialization training or is undergoing third r...
