Description:
The subject of the order is the provision of medical services in the field of: 1) occupational medicine health services: a) preventive medical examinations (periodic and check-ups) of employees of the IDEAS Research Institute and preliminary examinations of persons employed or transferred jobs where factors harmful to health or burdensome factors occur, along with the issuance of medical certificates confirming the absence of health contraindications to work or health contraindications to work in a specific position work, b) occupational risk assessment, c) health prevention, d) consultations and diagnostics in the field of occupational diseases, e) protective vaccinations, f) medical rehabilitation, 2) additional health services, i.e. comprehensive, specialized medical care for employees of the IDEAS Research Institute, implemented as part of packages. A detailed description of the subject of the order (OPZ) along with a description of additional medical services can be found in Annex No. 1 and 1A to the SWZ. Other terms of execution of the order include: draft provisions of the contract constituting Annex No. 2 to the SWZ. As part of the proceedings in question, the Contractor should submit: 1) an offer form - the form template is Annex No. 3 to the SWZ (the form must be submitted in the original and signed by a qualified electronic signature or trusted signature or personal signature). In the event of any discrepancies, the Ordering Party will consider the declaration submitted on the Offer Form (Appendix No. 3 to the SWZ) to be appropriate. 2) power of attorney...
