Providing medical services in the field of preventive health care and other services in in the field of occupational medicine for police officers and employees of the Police of the West Pomeranian Voivodeship in the cities of Szczecin and Koszalin
1. The subject of the order is the provision by the Contractor of services in the field of preventive health care and other services in the field of occupational medicine for officers and employees of the Police of the West Pomeranian Voivodeship, issuing on this basis medical certificates on the lack or existence of contraindications to service/work in the position held and determining their type, including the performance of: periodic and control examinations for police officers and Police employees, examinations when changing service/working conditions, preliminary examinations of candidates for employment in the Police (initial examinations of employees), periodic examinations, sanitary and epidemiological examinations, examinations of scuba divers (without a hyperbaric chamber), examinations of inland navigation helmsmen and examinations laboratory and diagnostic equipment for officers leaving and returning as part of the United Nations Mission.
2. The subject of the order is divided into 2 parts:
1)Part No.: I - name: provision of medical services that will be provided in a medical facility located in the city of Szczecin
2) Part No. II - name: provision of medical services that will be provided in a medical facility located in the city of Koszalin
