Description:
1. The subject of the order is the provision of occupational medicine services for employees of the Capital City Branch. Warsaw.2. The subject of the order includes: 1) carrying out preventive examinations of the Ordering Party's employees: initial, periodic and control examinations, as well as sanitary and epidemiological regulations provided for in: a) the Act of June 26, 1974, the Labor Code, hereinafter referred to as the Labor Code, b) the Act of December 5, 2008 on the prevention and combating of infections and communicable diseases in humans (hereinafter referred to as the Act), c) the regulation of the Minister of Health and Social Welfare of May 30, 1996 on medical examinations of employees, the scope of preventive health care for employees and medical certificates issued for the purposes provided for in the Labor Code, in accordance with the issued order by the Ordering Party (hereinafter referred to as the Regulation), 2) performing laboratory tests ordered by an occupational medicine doctor, 3) issuing medical certificates for the purposes specified in the Labor Code and implementing regulations, in accordance with the applicable templates, in two ways copies, 4) participation of the doctor providing health care in the work of the occupational health and safety committee at the request of the Ordering Party...
