- 1. The subject of the order is the provision of occupational medicine services, initial, periodic and control preventive examinations for employees of the Wielkopolska Voivodeship Office in Poznań and the Delegations in Kalisz, Konin, Leszno and Piła for 2026 and 2027 according to the following parts: 1) Part I provision of services with scope of occupational medicine, preliminary, periodic and follow-up preventive examinations for employees of the Greater Poland Voivodeship Office in Poznań;2. A detailed description of the subject of the order is included in Appendix No. 4 to SWZ.3. The scope and frequency of preventive tests are determined each time by the attending physician preventive examinations. Therefore, the final decision on the scope and frequency of tests is made by the occupational medicine doctor.4. The ordering party guarantees the execution of the order in the amount of min. 60%. The Contractor is not entitled to any claims for failure to complete the entire order.5. The Ordering Party in accordance with point 6 point 4 of the Regulation Minister of Health and Social Welfare of May 30, 1996 on medical examinations of employees, the scope of preventive health care for employees and medical certificates issued for the purposes provided for w...
