Providing occupational medicine services for policemen and police employees of the province Lesser Poland - Municipal Police Headquarters in Nowy Sącz/Tarnów, District Police Headquarters in Bochnia/Brzesko/Limanowa. 1) The subject of the order is the provision of occupational medicine services for police officers employees of the Municipal Police Headquarters in Nowy Sącz/Tarnów, the District Police Headquarters in Bochnia/Brzesko/Limanowa performing official duties in the area of operation of the above-mentioned Police unit, as well as, if necessary, police officers and employees of other police units in the province Lesser Poland, the Ordering Party requires the provision of services in the cities indicated in Chapter V - Qualification of Contractors. 2) Detailed description of the subject of the order and expected quantities of services are included in Annex No. 2 to the SWZ - price form and Annex No. 4 - draft contract. 3) The service will be provided depending on the needs of the Ordering Party. The expected quantities of medical services given in Annex 2 to the SWZ (price form) are values forecasted. These values are indicated only for the purpose of selecting the most advantageous offer by the Ordering Party and do not constitute an obligation of the Ordering Party towards the Contractor. At the same time, the Ordering Party guarantees the performance of the contract at a minimum level. 70% of the contract value specified in §14 section 1. 4) The contractor guarantees that all tests will be completed within a maximum of 3 business days, taking into account the day the policeman/employee reports to perform the test...
