Description:
The subject of the order is the provision of rehabilitation services for 16 participants of the Senior+ Day Care Home in Opiny in the form of individual and group classes.1) Participants of the Senior+ Day Care Home in Opiny are people who need support in everyday functioning and who, due to their health condition or disability require care or support due to the inability to perform at least one of the basic activities of daily living.2) the provision of the service consists in particular in: a) conducting individual physical rehabilitation with DDS+ participants, in accordance with doctors' recommendations, b) conducting motor and sports activities: - active exercises, - passive exercises, - supporting exercises, - resistance exercises, - weight-bearing exercises, - breathing exercises, - exercises with equipment available in DDS+, c) conducting massages according to the individual needs of DDS+ patients, d) keeping documentation, e) cooperation with a care, therapeutic and design team, f) diagnosing dysfunctions of patients' movement organs, g) explaining to participants the need and purposefulness of the exercises performed, h) periodic analysis of activities resulting from Individual Action Plans.3) Information...
