Description:
The subject of this order is the provision of medical services in the field of: 1) mandatory occupational medicine examinations, carried out in accordance with the Regulation of the Minister of Health and Social Welfare of May 30, 1996 on medical examinations of employees and the scope of preventive health care over employees and medical certificates issued for the purposes provided for in the Labor Code and the Act of August 27, 1997 on vocational and social rehabilitation and employment of disabled persons, which include: a) preliminary examinations of employees employed and persons employed in order to complete an internship, b) periodic examinations of employees, c) control examinations in the event of an employee's inability to work lasting longer than 30 days due to illness, d) examination of the driver of a car with a permissible total weight of up to 3.5 tons (category B driving license required); 2) participation of an occupational medicine doctor in meetings of the Occupational Health and Safety Committee appointed by the Ordering Party as part of the value of the concluded contract;3) additional tests listed in point 5 of the table.
