Description:
The subject of the order is the service of relocation and collocation of two servers along with the necessary telecommunications infrastructure. A detailed description of the subject of the order is included in Annex No. 1 to the SWZ (hereinafter referred to as OPZ). The ordering party requires the submission of:1. Offer form - the form template is attached as Annex No. 2 to SWZ (in the event of any discrepancies, the Ordering Party will consider the declaration submitted on the Offer Form (Annex No. 2 to the SWZ) to be appropriate); 2. Powers of attorney established to represent the Contractor or Contractors applying for the award of a public contract - if the tender is submitted by representative;3.Contractor's declaration of not being subject to exclusion from the proceedings - a template of the declaration is attached as Annex No. 3 to the SWZ;4.Contractor's declaration of meeting the conditions for participation in the proceedings - a template of the declaration constitutes Annex No. 4 to the SWZ;5.Commitment of the entity providing the resources or other subjective means of evidence confirming that the Contractor, when implementing the order, will have the necessary resources of these entities at its disposal. A contractor who refers to the resources of the entity providing the resources in order to demonstrate the lack of grounds for exclusion and compliance, in the scope referring to their resources, conditions of participation in the proceedings, also submits the documents indicated in the above-mentioned pts. 3 and 4 (Appendix No. 3 and 4 to the SWZ) - if applicable; 6. Declarations of Contractors acting jointly, which will indicate which elements of the order will be performed by individual Contractors - if refers to...
