Description:
The subject of the contract is the provision of a sanitary transport service in the field of transporting patients from the place of residence to the Criminal Wound Treatment Clinic at the Employer and Return. The service will be carried out by ordinary (T) or basic (P) transport. The contractor is obliged to providePatient transport services from Monday to Friday throughout the entire period of the contract during the working hours of the Clinic Clinic of Chronic Wounds. A detailed description of the subject of the contract includes: Annex 1 to SWZ - price form and Annex 1 A to SWZ - description of the subject of the contract. DetailedThe conditions and rules for the implementation of the subject of the contract regulate the provisions of the "contract template" - Annex 5 to SWZ. The deadline for completing the order: 18 months from the date of signing the contract. The number of orders for transport will result from the legitimate needs of the Employer.
