Brief description of the subject of the order: The subject of the order includes the provision of cremation services for bodies and remains for the needs of Zakad Usug Funeral Sp. z o. o. in Ostrowiec w. in the material scope and in the estimated quantities given in Chapter 2 of the SWZ, Estimated size of the order during the term of the contract -amounts to approximately 280 pieces of cremation and includes: a) cremation of the remains of an adult - 275 pieces, b) cremation of corpses or remains after exhumation - 2 pieces, c) cremation of children under 6 years of age - 1 piece, d) cremation collective pods and bodies of stillborn children - 2 pieces, the scope of the order includes the following activities:, a) Collecting and transporting the coffin with the body from the hearse to the crematorium premises, b) Creating cremation of the bodies of deceased persons or remains after exhumation and collective cremation of bodies and embryos, respecting the bodies and in accordance with applicable regulations, c ) free accessthat the family of the deceased person will be provided with an Observation Room, where the family of the deceased person will be able to observe the moment the body is placed in the cremation furnace, d) issuing an urn with the ashes and appropriate documents confirming the cremation.
