Description:
The subject of the contract is the provision of a service in the field of patient transport by sanitary transport with a T type syndrome, as described by the subject of the contract and with relevant contractual provisions and the instructions in force at the Employer -io 4-53-13, constituting Annex No.12 to this SWZ.
A detailed description of the subject of the contract is attached No. 4 to SWZ (OPZ)-description of the subject of the contract.
3.2 The detailed conditions and principles of the implementation of the subject of the contract are additionally specified in the contractual terms that is Annex 5 to SWZ "template of the contract".
Before signing the contractor's contractTogether applicants for the award of the contract (in the case of choosing their offer as the most advantageous) they will present the contracting authority regulating the cooperation of these Contractors.
Within 2 days before signing the contract, the Contractor will provide the Ordering Party:
- copies of communication policies andvalid registration documents of the above vehicles,
- copy of the civil liability policy in the scope of conducted activities related to the subject of the contract for an amount of not less than PLN 100,000.00
- list of persons who will provide the contracting authority service together with a photocopy of their rights confirmedFor compliance with the original
