Description:
1. The subject of the contract is the provision of sensory integration therapy for children as part of a comprehensive support program for families "for life". Therapy services will be provided in the form of individual classes with children from birth to their school education.
For one The time of therapy is taken full time (60 min.).
441 of the PZP Act consisting in the unilateral entitlement to increase the scope of the contractor's benefits, consisting of Provision of early development support services for children as part of a comprehensive support program for families "for life", by increasing the number of hours of therapy to implementation, in accordance with the provisions of point 2.6 SWZ.
The maximum value of the option (estimated maximum number of therapy hours) is: 400 hours

The contractor will not be entitled to any claims for the unused part of the contract, except the claim for payment for the service rendered.
