Description:
1. The subject of the contract is the provision of a physiotherapy service for children as part of a comprehensive support program for families "for life". Therapy services will be provided in the form of individual classes with children from birth to their school education.
For one hour of therapy A full clock hour is taken (60 min.).
2. The subject of the contract was divided into 2 parts according to the expected number of hours (H) Services:
- For part 1 of the procedure, the estimated minimum number of hours is 100 hours
- For part 2 of the procedure, the estimated minimum number of hours is 50 hours
3. The contracting authority has the right to the option referred to in art. 441 of the Public Procurement Law, consisting in the unilateral entitlement to increase the scope of contractor's benefits, consisting in providing early development support services for children as part of a comprehensive support program for families "for life by increasing the number of hours of therapy to implementation, in accordance with the provisions of point 2.6 SWZ.
The maximum value of the option (estimated maximum number of therapy hours) is:
- For part 1 of the procedure, the estimated maximum number of hours is 200 hours
- estimated maximum amount for part 2 of the procedure hours is 100 hours
This is the maximum number of hours that the Employer can order for implementation as part of a given part.
