- 1. The subject of the order is the provision of physiotherapy services for children as part of the comprehensive support program for families "For Life". Therapy services will be provided in the form of individual classes with children from birth until they start school.
For one hour of therapy a full clock hour (60 minutes) is assumed.
2. The subject of the order was divided into 4 parts according to the expected number of hours (h) of the service:
- for part 1 of the procedure, the estimated minimum number of hours is 250 h
- for part 2 of the procedure, the estimated minimum number of hours is 500 h
- for part 3 of the procedure, the estimated minimum number of hours is 250 h
- for part 4 of the procedure, the estimated minimum number of hours is 300 h
3. The allocated number of hours will depend on the number of children actually qualified for the program. For unused parts contract, the Contractor will not be entitled to any claims, except for payment for the service provided.
4. The responsibilities of a specialist conducting individual physiotherapy for children include:
- after appointing a team of specialists for a specific child, establishing directions and a schedule activities undertaken in the field of early assistance and family support,
- establishing cooperation with the facility where the child is subject to other therapeutic activities in order to ensure coherence of interactions,
- developing and implementing an individual project together with the child and his family program... - Documents:
