Description:
1. The subject of the order is: provision of social neighborhood services for the needs of the project entitled Creation of the Social Services Center in Ostrowiec witokrzyski, including: Providing adult participants of the project with support in the form of neighborhood help,2. Services include: Providing adult participants of the project with support in the form of neighborhood assistance. The total period of coverage of services: from signing the contract to October 31, 2027 in three time frames: a) in the period from signing the contract to March 31, 2026 - 2,800 hours, i.e. 48 environments (including 46 communities for 3 months in neighborhood help - an average of 20 hours a month), and 2 communities (for 1 month, an average of 20 hours a month)b) in the period from April 1, 2026 to January 31, 2027 - 1,600 hours, i.e. 20 communities (1 community is in the neighborhood for 4 months help neighborhood - on average 20 hours a month), c) in the period from February 1, 2027 to October 31, 2027 - 1,600 hours, i.e. 20 environments (1 community is in neighborhood assistance for 4 months - an average of 20 hours a month). Total number of hours of service - 6,000 (1 hour 60 minutes - to time of providing services at the client's place, travel to or from the place of service is not included)
