(wards of MGOPS Wiele) from January 2, 2026 to December 31, 2026, pursuant to: a) Art. 8 and 9 of the Act of August 19, 1994 on mental health protection b) Art. 18.1 point 3 of the Act of 12/03/2004 on social assistance. and according to scope specified in: c) Min. Reg. Polityka Spo. of 22/09/2005 regarding specialist care services (Journal of Laws No. 189, item 1598, as amended). Estimated average number of service recipients: 8. Estimated number of hours of services - 4,100 hours. during the mandatory period contract. The number of hours and people covered by the services may change and cannot be determined precisely. due to the specificity of the order, which is characterized by changing customer needs due to their changing health condition. The number of hours of services provided is their estimated amount, and the Ordering Party will pay for the hours actually provided. Detailed description of the subject of the order.
