Description:
The subject of the order is the provision of care services at the place of residence in the City of Jzefow (pursuant to Article 50 of the Act of March 12, 2004 on social assistance) for the residents of Jzefow, in accordance with the scope established in the administrative decision granting the benefit. The services are to be provided on working days, on public holidays and non-working days, from 6:00 a.m. to 10:00 p.m. The estimated number of hours planned for the duration of the basic order is approximately 10,800. Due to the specificity of the subject of the order, the Ordering Party reserves the right to significantly change the number of people using the services care during the term of the contract. By the hours of service provision, the ordering party understands the hours provided at the place of residence of the MOPS residents in Jzefow. The number of hours and people is an estimate and may change due to the specificity of the order, which is characterized by changing needs due to to the changing health status and life situation of people covered by care services.
