- The subject of the contract is the provision of care services referred to in the Act of 12 March 2004 on social assistance (Journal of Laws 2024 item 1283, as amended), hereinafter referred to as "services", towards the charges of the centers/Social Welfare Center of the Capital City of Warsaw who, due to age, illness or other causes,They require the help of other people in their place of residence, and they are devoid of it. For the implementation of the subject of the contract, the Purchaser will provide the Contractor with the possibility of logging in and using the e-care system. The system includes, among others e-care web application (for coordinators) and the Mobile M-Opieka application(for carers). Order carried out by 8 parts:
part 1 covering the districts: Rembertow, Wawer, WesoΕa in the dimension of 82,800 hours in the full period of order processing, with:
estimated average monthly number of hours of services will be 6,900 hours;
expected monthly averageThe number of recipients of services will amount to 242 people;
minimum dimension of the hours of services provided in the full period of order performance, will be 66 240 hours.
Part 2 covering the Praga-PoΕudnie district in the amount of 247,000 hours in the full period of order processing, with:
estimated monthly averageThe number of hours of services will be 19,000 hours;
The expected average monthly number of recipients of services will be 716 people;
minimum dimension of the hours of services rendered in the full period of order processing, will be 197 600 hours.... - Documents:
