Description:
1. The subject of the contract is the performance of the task entitled: "The provision of medical services in the field of occupational medicine and outpatient medical care for IMGW-PIB employees and their family members." 2. A detailed description of the subject of the contract can be found in Annex 1 to SWZ - description of the subject orders. 3. List of jobs is Annex 1A to SWZ. 4. Employment structure is Annex 1B to SWZ. 5. Specification of exposure and exposure to risk at workplaces is Annex 1C to SWZ.
1. The subject of the contract is the performance of the task: "Providing services medical in the field of occupational medicine and outpatient medical care for IMGW-PIB employees and their family members. "2. A detailed description of the subject of the contract can be found in Annex 1 to SWZ-description of the subject of the contract. 3 The list of jobs is Annex 1a to SWZ. 4. Structure. Employment is Annex 1B to SWZ. 5. Specification of exposure and exposure to risk at workplaces is Annex 1C to SWZ. 6. As part of the implementation of the subject of the contract, the Contractor will provide: 1) to all employees of the contracting authority medical care in the field of occupational medicine in the form Occupational Medicine package (sample name); 2) the declared entitled to the Ordering Party and their family members medical care in the field of outpatient medical care in the form of a subscription as part of medical packages.
