Description:
1. The subject of the order is the provision of medical services in the field of occupational medicine and additional health services to the Ordering Party's soldiers and employees in the premises located at the Ordering Party's headquarters at ul. gen. Sylwestra Kaliskiego 15 in Warsaw (building no. 9 roomNo. 5) with a total area of 117.31 m2 including the adaptation of the above-mentioned premises to provide the service. The scope of occupational medicine tests and additional health services will be carried out in accordance with applicable regulations, in particular:
a) Act of April 15, 2011 on medical activities (Journal of LawsLaws of 2024, item 799, i.e.);
b) Act of June 27, 1997 on occupational medicine services (Journal of Laws of 2022, item 437, i.e.);
c) Act of June 13, 2019 on conducting business activities in in the field of production and trade in explosives, weapons, ammunition and products and technologymilitary or police purposes (Journal of Laws of 2023, item 1743, consolidated text);
d) Act of March 11, 2022 on the defense of the homeland (Journal of Laws of 2024, item 248, consolidated text).
2 . The scope of the service is specified in Annex No. 1A to SWZ.
3. Guidelines for adaptation works in the shared premises at ul. Gen. Sylwestra Kaliskiego 15 were specified in Annex No. 1B to the SWZ.
