The subject of the order is the provision of occupational medicine services for employees/officers of the Tax Administration Chamber in Warsaw as part of Part 1 of the subject of the order - RADOMS REGION.2. Detailed description of the subject of the order, scope and method and requirements for the implementation of the subject of the order, define:2.1. sample contract with annexes (including, among others, OPZ - Za. No. 1 to the contract template, List of IAS units within the individual parts of the subject of the contract, Za. No. 6a) to the contract template), which constitutes Annex No. 2 to SWZ, 2.2 nw. an annex that specifies the estimated amounts and types of tests, i.e.: Annex No. 4a) to SWZ - Detailed offer form for Part 1 of the subject of the order - RADOMS REGION. This form is auxiliary to the valuation of the gross price of the offer.3. Pursuant to section 6 1 of the contract template, the Contractor must have civil liability insurance for the entire duration of the contract. with running a business (third party liability insurance) for a guarantee sum of a value not less than the gross value of the basic order of the contract for the performance of the subject of the order for one and all events.
