1. The subject of the order is the PROVISION OF CARE SERVICES AT YOUR PLACE OF RESIDENCE IN ACCORDANCE WITH ART. 50 ACT OF MARCH 12, 2004. ON SOCIAL ASSISTANCE FOR PEOPLE LIVING IN THE MUNICIPALITY OF CIKOWICE, BENEFICIARIES OF THE MUNICIPAL SOCIAL ASSISTANCE CENTER IN CIKOWICE.2. Scope of the order:1) Scope of the order covers the provision of care services in a total amount of 7,440 hours for 35 people in the period from January 1, 2026 to December 31, 2026. 2) The planned average number of hours of services provided per month is 620. 3) The basis for settlement with the Contractor will be 1 (one) working hour (60 minutes), to which not included travel time/reach time to the patient.4) Services are to be provided on working days, holidays and holidays. The number of hours of services provided will depend on the scope of services, the number of clients and their needs and will change during the duration of the contract.5) Provision of services for a specific person After receiving from the Ordering Party, the Contractor will start a catalog of care services or specialized care services, which will specify the date of commencement of the provision of services, the scope and frequency of activities performed. ...
