- 1. The subject of the contract is the provision of services in the delivery of disabled children residing in the Praga-Połnoc district of the Capital City of Warsaw to kindergarten, kindergarten department at primary school, a different form of pre -school education, as well as for primary or secondary schooland other facilities listed in art. 2 points 7 of the Act of December 14, 2016 Educational Law (Journal of Laws of 2024, item 737, as amended) on the "Dom -Szkoła -Dom" route and in the scope of taking care of children during delivery in the period from September 1, 2025 to August 31, 2026
2. The transport will be covered byDisabled children, who have a current certificate on the need for special education covered by pre -school education, meeting the obligation of annual pre -school preparation, school obligation or the obligation to study.
3. The delivery will take place to kindergartens, schools and educational institutionsoperating in the Capital City of Warsaw Warsaw, and in justified cases referred to in art. 127 para. 15 of the Act of 14 December 2016 Educational Law (Journal of Laws of 2024, item 737, as amended) also to the nearest special school or a center located in the poviats neighboring the Capital City ofWarsaw.
4. A detailed description of the subject of the contract and requirements for the method of its implementation are determined by:
a) description of the subject of the contract (together with a list of disabled children covered by delivery) - Annex 12 to SWZ,
b) Designed provisions of the contract - Annex No. 11 toSWZ
