Description:
1. The Ordering Party commissions and the Contractor accepts the subject of the contract (order), which is the provision of services by one assistant for an elderly person to the participants of the project called: Wielkopolska care telecenter (hereinafter referred to as the Project) in their place of residence in the manner and on the terms specified in this agreement.2. By the Project participants referred to in section 1, should be understood as elderly people over 60 years of age from the city of Turku who, due to age, health condition or disability, require care or support in everyday functioning, hereinafter referred to as Participants or Participant.3. The scope of duties of an elderly person's assistant includes, among others: a. Development and implementation of a work plan for older people in consultation with the coordinator of the Senior Support Center and a social worker. b. Periodically assessing the situation of the elderly person, at least every year, i forwarding this assessment to the coordinator of the Senior Support Center.c.
