- Brief description of the subject of the order: I. The subject of the order is the provision of transport services for the Polkowice Center of Health Services ZOZ S.A., mainly in the Lower Silesian Voivodeship, in particular the commune of Polkowice, Lubin, Gogw:, II. Detailed description and method of order fulfillment: within the scopeprovision of transport services, 1) Services will be provided from Monday to Sunday from: 7.00-20.00, 365 days a year; , 2) The contractor has at least 1 passenger car (5-seater) and, upon request, 1 passenger-delivery bus type vehicle (adapted to transport furniture, large boxes, cylinders withlinen 2, 5, 10 liters, medical equipment, etc.). Cars should be kept impeccably clean. , 3) Waiting time for passenger transport is no more than 15 minutes. , 4) Services will be provided on a non-cash basis based on completed transport cards, on which the Ordering Party's employeewill confirm the execution of the transport service on an ongoing basis.
