Description:
The subject of the order is the provision of psychological support services: 1. For home hospice residents (people aged 65 and older) - 24 people per month for 3 months each; 2. For informal caregivers of hospice patients (helping caregivers in coping with emotions, stress and challenges related to caring for dependent persons).Care provided at the patient's place of residence.Psychological services: 1) 24/7 access to services provided by psychologist. 2) For each person needing support, 2 consultations will be provided per month, 3) For informal caregivers of hospice patients, 1 consultation will be provided per month 4) Services will be supported through the use of remote channels to contact the patient telemedicine.5) Cooperation within the therapeutic team in terms of patient qualifications and creation of an individual patient care plan (together with a doctor, nurse, rehabilitator, and project administrator).6) The bidder provides its own transport to mentee
