Description:
The subject of the contract is the delivery of 5 medical ambulances type C with 4x4 drive meeting the requirements of the PN standard: EN 1789 or equivalent standard together with medical equipment that meets the requirements of the PN EN 1865 standard or a standard equivalent to the needs of the Provincial Emergency Service Station in Przemyśl SP ZOZ, ul. Juliusz Słowackiego 85a, 37 - 700 Przemyśl.
The subjective evidence
The contractor applying for the award of the contract is obliged to submit along with the offer the following evidence confirming that the offered delivery meets the requirements specified by the Employer, i.e. A statement confirming that the offered vehicle and medical equipment corresponds to the requirements specified by the Employer in the description of the subject of the contract, i.e. detailed quality, technical and functional data indicating the parameters of the offered vehicle required by the Employer, in accordance with the attachment No. 2 to SWZ (statement of technical parameters that is also a detailed description of the subject of the contract)
The evidence in question referred to in item 3.4 liters and should be in an electronic form provided with a qualified electronic signature.
The contracting authority does not provide for the possibility of supplementing the subject of evidence, due to the fact that it serves to confirm compliance with the features, constituting the basis for verifying the substantive correctness of the offer
