Description:
1. The subject of the order is the purchase and delivery along with the installation of a stationary X-ray machine for the Katowice Oncology Center, in accordance with Annex No. 2 to the SIWZ - Price Offer.
2. The technical and functional parameters of the equipment are specified in Annex No. 4 to the SWZ.
The order includes also:
1) transport of the camera to its destination,
2)installation, commissioning,
3) delivery of software in accordance with Annex 4 to the SWZ together with perpetual licenses,
4) connecting the delivered equipment to the PACS/RIS system owned by the Ordering Party
and configured correctly work with this system (the equipment is to download data from the DICOM Worklist issued by the PACS/RIS system and send the tests back to the PACS system; the equipment sends MPPS messages to the PACS/RIS system),
5) staff instruction and provision of medical equipment for use along with all necessary certificates, certificates,
6) cooperation with the Ordering Party in the preparation of documentation in order to obtain all necessary permits/permits to use the subject of the order, including the permit of the State Provincial Sanitary Inspector in Katowice.
7) 24-month warranty from the date of signing by authorized representatives of the Ordering Party and the Contractor of the "Final acceptance report of the subject of the order".
8) 36-month post-warranty service counted from the day following the end of the warranty period
