Description:
1. The subject of the contract is the purchase and delivery along with the installation of a computed tomograph for planning for the Katowice Oncology Center, in accordance with Annex 2 to the ToR - price offer.
2. The technical and functional caps of the equipment are specified in Annex 4 to SWZ.
OrderIt also includes:
1) transport of a computed tomograph (hereinafter in SWZ equipment/camera) to the destination,
2) installation, startup,
3) connecting the supplied equipment to the PACS/RIS system owned by the Employer and configuring for proper operation with this system (the equipment is to downloadData with Dicom Worklist issued by the PACS/RIS system and send research to the PACS system; The equipment is sent by MPPS messages to the PACS/RIS system),
4) personnel instruction and handing over medical equipment for use together with all necessary approvals, certificates,
5) cooperation with the Employer inPreparation of documentation in order to obtain all necessary permits/permits for the use of the subject of the contract, including the permission of the State Atomicism Agency.
6) 24 -month warranty calculated from the date of signing by the authorized representatives of the Ordering Party and the contractor "Protocolfinal receipt of the subject of the contract ".
7) 36 -month -old post -warranty service counted from the day after the end of the warranty period
The Ordering Party reserves the right to annul the procedure pursuant to Article 257 pzp.
