- 1. The subject of the order is the delivery of a 3D diagnostic and surgical set.2. A detailed, technical description of the subject of the order is attached as Appendix No. 5 to the SWZ. 3. The offered 3D laparoscopic diagnostic and surgical set must be a medical device within the meaning of the regulations: o Regulation (EU) 2017/745 of the European Parliament and of the Council of 5 April 2017 on medical devices (MDR), o the Act of 7 April 2022 on medical devices (Journal of Laws 2024.1620, consolidated text), o and other implementing provisions applicable to the placing on the market and use of medical devices.4. The product must have: o CE marking, confirming compliance with the requirements of the MDR regulation, o EU Declaration of Conformity, issued by the manufacturer or his authorized representative, o Instruction manual and marking in Polish, o Catalog card or technical description confirming the parameters of the offered product. set.5. All elements of the set must be brand new, free from defects, admitted to trading in the European Union and come from one manufacturer or be fully assembled. compatible.
