Description:
1. The subject of the order is the purchase and delivery of medical devices for the Air Ambulance Service - syringe infusion pumps. The Ordering Party requires that: 1) the medical equipment be approved for trading and use in the territory of the Republic of Poland in accordance with the requirements of the Medical Choices Act. 2) the delivered medical devices were approved for use in air transport. 3) the medical devices could be mounted in a certified wall mount mounted in the medical cabin of the EC 135 helicopter (P2, P2+, P3). 4) the delivered medical devices were brand new and had at least 24-month warranty period counted from the date of signing the acceptance protocol for medical devices without reservations.5) The contractor will provide instructions for medical devices in paper and electronic versions in accordance with the OPZ.6) Authorized service in the territory of the Republic of Poland will be provided.2. Detailed description of the subject of the order is included in the Description of the subject of the order (OPZ Form of required parameters), which constitutes Annex No. 1 to the SWZ, and in the Draft Provisions of the Agreement (PPU), which constitutes Annex No. 3 to SWZ.
