Description:
1. The subject of the contract is the purchase and delivery of medical equipment and apparatus: ultrasounds - 3 pcs and intraoperative ultrasound - 1 item for the Katowice Oncology Center, in accordance with Annex 2 to SWZ - price offer.
2. Technical and functional caps of the equipment have been determined inAnnex 4 to SWZ.
The order also includes:
1) camera transport (hereinafter referred to as the equipment/camera) to the destination,
2) installation, startup,
3) providing the software in accordance with Annex 4 to SWZ together with indefinite licenses,
4) connecting the supplied equipmentto the PACS/RIS
system owned by the Employer and configuring for proper operation with this system (the equipment is to download data from Dicom Worklist issued by the PACS/RIS system and send research to the PACS system; the equipment sends MPPS messages to the PACS/RIS system),
5) Personal instruction Ihanding over medical equipment for use together with all necessary approvals, certificates,
6) 24 -month warranty calculated from the date of signing by the authorized representatives of the Ordering Party and the contractor "The final protocol of the final acceptance of the subject of the contract".
7) 36 monthly post -warranty servicecounted from the day after the end of the warranty period;
The contracting authority reserves the right to annul the proceedings pursuant to art. 257 PZP.
