- 1. The subject of the contract is: successive delivery to the headquarters of the Ordering Party - pharmacy and/or pharmacy department - (together with transport and unloading) of substitute medicinal products in assortment and quantitatively in the assortment and price form constituting annex and perceiveOrders and implementation conditions are specified in:
a) the offer form, which is Annex 1 to SWZ,
b) The detailed scope of the subject of the contract is set out in the assortment and price form, which constitutes Annex 2 to SWZ,
c) The draft contract, the template of which is Annex No. 8 to SWZ.
3. Miejsc.The supply will be:
A.Szpital Pulmonological and Reumatology in Kup, ul. Karola Miarki 14, 46-082 Buy-
premises of the hospital pharmacy-III pr. Main building of the hospital in Kup,
b.szpital rehabilitation in the room, ul. NamysΕowska 22, 46-034 Room-Pharmacy Department-the main building of the hospital in the room.The order of medicines will be provided with an indication of the place of delivery, i.e. delivery to a hospital pharmacy at Buy or delivery to the pharmacy department in the room.
5. The subject of the contract should be successively carried out, in assortment positions and quantities resulting from the current needs of the Ordering Party, afterPrevious notification of the order to the contractor's email address. Funding the goods to the warehouse lies with the contractor. The contracting authority expects the supply of individual parties to the indicated locations in para.2.
