• Description:
• Purchase and delivery of medicines for the Air Ambulance Service. 1. The subject of the order is the successive sale and delivery of medicines to branches and branches as well as the Medical Logistics Department of the Air Ambulance Service. Detailed description of the item the order constitutes the Product and Price Form (Appendix No. 2 to the SWZ). The Ordering Party reserves that the given quantities of the product are estimated quantities which may change during the term of the contract. The Ordering Party reserves the right to make changes the quantity of the ordered assortment as part of the total value of the contract. The Ordering Party reserves the right to resign from purchasing part of the product range due to lack of demand. The Ordering Party requires that: 1) The medicines that were the subject of the order were approved for use trade and use in the territory of the Republic of Poland based on an entry in the Register of Medicinal Products Authorized for Marketing in the territory of the Republic of Poland. 2) The delivered assortment was new and at least 12 months old fitness for use counted from the date of its delivery to the Ordering Party. (Parameter assessed as part of the offer evaluation criteria). 3) The deadline for each delivery was no longer than 14 days from the date of placing the order.
