- The subject of the order is the supply of perfusion fluids with accessories to the UCKWUM hospital in Warsaw, UCK WUM SKDJ pharmacies at ul. Lindleya 4 and UCK WUM CSK Pharmacy at ul. Nielubowicza 5 divided into 3 parts Detailed description of the subject of the order constitutes Annex No. 2 to the SWZ - Assortment and price form. The contractor is obliged to deliver the subject of the contract as indicated in the order, to: -UCK WUM SKDJ pharmacies at ul. Lindley 4 -UCK WUM CSK pharmacies at ul. Nielubowicza 5 on based on individual orders successively placed by the Ordering Party. The validity period is: min. 12 months from the date of delivery to the Ordering Party. The deadline for completing the order is: 12 months from the date of conclusion of the contract or until maximum exhaustion remuneration specified in the contract.
