Description:
1. The subject of the order is the supply of reagents for microbiological tests along with the lease of analyzers, necessary devices and equipment, applicable to Parts No. 1, No. 2, No. 3, No. 4. A detailed description of the subject of the order can be found in the "Assortment and price form", which is an Annex. No. 3 to the SWZ and in the "Description of the subject of the order", constituting Annex No. 4a, No. 4b and No. 4c to the SWZ. the medical device was registered as a medical device and met the requirements of the Act of April 7, 2022 on medical devices (Journal of Laws 2024.1620 z 2024/11/05) and Regulation (EU) 2017/745 of the European Parliament and of the Council of 5 April 2017 on medical devices (Medical Devices Regulation) and had the registration certificates, attestations, certificates and other documents required by law to authorize the subject of the order for sale and use in Poland and, if applicable, met the requirements of Regulation (EU) 2017/746 of the European Parliament and of the Council of 5 April 2017 on in vitro diagnostic medical devices and repealing Directive 98/79/EC and Commission Decision 2010/227/EU.
