- The subject of the order is the successive deliveries of medicinal products to the Hospital Pharmacy of the Provincial Specialist Hospital for the Neurologically and Mentally Ill, SPZOZ in Ciborz. A detailed description of the subject of the order is included in the Price Form - Annex No. 2.1-2.16 to the SWZ. Any Contractor may submit only one offer for the entire order or a given task/tasks in writing under pain of invalidity. If the Contractor submits more than
one offer for the entire order or for a given task/tasks, all offers from this Contractor will be rejected by the Ordering Party. Order consists of 16 tasks. The Ordering Party allows the submission of partial offers. The Ordering Party will consider an offer submitted for all items in a given task to be a partial offer. The Ordering Party does not allow the submission of offers for single items. The Ordering Party stipulates that the offered subject of the order must belong to groups of medicinal products in accordance with Art. 2 section 32 and art. 40 of the Pharmaceutical Law Act of September 6, 2001 (consolidated text: Journal of Laws of 2025, item 750).
The offered products must have appropriate certificates or approvals allowing their use in health care in accordance with Polish law and European Union law. Medicinal products (drugs) must be entered in the Official List of Products
