Description:
1. The subject of the order is the purchase and successive delivery of drugs from drug programs, specified in parts for the University Clinical Hospital. Fryderyk Chopin in Rzeszow
2. A detailed description of the subject of the order and the Ordering Party's requirements are specified in the Price and Offer Forms Annexes No. 1 for individual parts that are an integral part of the SWZ.

The scope of the order includes the delivery of the order items in the quantities specified in the Price and Offer Forms for the period:

5. ORDER COMPLETION DEADLINE:
5.1 Purchase and subsequent delivery of order items:
Part no. 1 to no. 2 - for 12 months from the date of conclusion of the contract.
5.2 In Part No. 1 to No. 2, the Ordering Party allows for the possibility of changing the validity period of the contract concluded with the Contractor in the event of failure to complete the entire subject of the contract during its duration. In such a case, changing the contract requires signing an annex, provided that the Ordering Party unilaterally notifies the Contractor about the proposed extension of the contract before the expiry of the deadline for which it was concluded. zawar...
