Description:
The subject of the order is the purchase and successive delivery of heart stimulators divided into tasks.
Detailed requirements for the performance of the contract were specified in the template of the contract and the detailed technical specification constituting, respectively, Annex 5 to SWZ and Annex 2 to SWZ.
In scope of task No. 1 - 5, the delivery will take place by transferring the deposit equipment to the contracting authority on the basis of the transfer document. The deposit state (assortment and quantity) will be determined by the contracting representative. Information on equipment consumption will be provided with fax onon the basis of the consumption protocol within 3 business days from the date of consumption - the protocol template is Annex 2 to the provisions of the contract relevant to the parties. The consumption protocol is the basis for issuing an invoice. The invoice should be issued within 7 days from the date of receipt of the consumption protocol. After displayingInvoices through the Contractor, the equipment passes into the ownership of the Employer. Deliveries of the goods must take place from Monday to Friday from 08:00 to 14:00 to the Diagnostics and Intravascan therapy Center -pavilion number 9.
In scope of task No. 6 -7 Delivery will take place by transferEmployer by the Contractor of the Hospital Pharmacies by the Contractor - Pavilion number 11 - within max. up to 3 business days from the moment the order was placed by the authorized representative of the Ordering Party.5
