Description:
The subject of the order is the purchase and successive delivery of coronary angioplasty equipment for the needs of the Wolski Hospital broken down into tasks. Detailed requirements for the performance of the contract were specified in the contract template and the detailed technical specification constituting respectivelyAnnex 5 to SWZ and Annex 2 to SWZ. In terms of task No. 1, 2 - item 5,6,7,8,12,14,15,16, in the scope of the task 3 item 1-6, task 4, 5, 6, 7 items 2-7, Zad 8, task 9 item from 2 to 7, task 10, task 11 item 9-14 and 17.18, task 12, 13, the delivery will take place by transferring to the contracting authority by the Contractordeposit equipment based on the transfer document. The deposit state (assortment and quantity) will be determined by the contracting representative. Information on equipment consumption will be provided with fax on the basis of a consumption protocol within 3 business days from the date of wear - the protocol template isAnnex 2 to the provisions of the contract relevant to the parties. The consumption protocol is the basis for issuing an invoice. The invoice should be issued within 7 days from the date of receipt of the consumption protocol. After issuing the invoice by the Contractor, the equipment becomes the ownership of the Employer. Goods must be deliveredtake place from Monday to Friday from 08:00 to 14:00 to the Diagnostics and Intravascan therapy Center - pavilion number 9 in the scope of the task No. 2 - position 1,2,4,9,10,11,13 in the scope of the task 3 item from 2-7, task 7 item 1, croup 9 item 1, task 11 item 1-8, task 14 - delivery will take placein ...
