In accordance with the description of the subject of the contract specified in the terms of reference (SWZ) and on the terms specified in the draft provisions of the contract (PPU), which constitute an annex to the SWZ.

Pursuant to Art. 105 and 106 of the Public Procurement Law in order to confirm compliance of the offered supplies with the requirements, features or criteria specified in the item description order or offer evaluation criteria or requirements related to the implementation of the order, the contracting authority requests the contractors to submit the following documents:
A valid marketing authorization in the territory of the Republic of Poland, valid on the date of submission of offers, issued by the President of the Registration Office Medicinal Products, Medical Devices and Biocidal Products or a valid marketing authorization issued by the Council of the European Union or the European Commission on the date of submission of offers.
