Description:
1. The subject of the order is: successive deliveries of consumables for the needs of the Ordering Party's Hemodynamics Laboratory.
2. Brief description of the subject of the order:
1) a detailed description of the subject of the order in terms of the required parameters regarding the task / part of the order no. 2 has been detailedlisted in Annex No. 2 to the SWZ;
2) a detailed description of the subject of the contract in terms of the required parameters regarding tasks / parts of the contract No. 1, 3 and 4 is specified in Annex No. 2a to the SWZ.
3. A detailed list of the quantities of consumable materials can be found in Annex No. 2 to the SWZ -assortment and price form.
4. The Ordering Party allows the submission of partial offers divided into four parts of the order / packages / tasks according to the following division:
1) part of the order No. 1 - stents;
2) part of the order No. 2 - guidewires and catheters;
3) part of the order No. 3 - set to Drpericardial enamage;
4) part of order no. 4 - lithotripsy system.
1. The subject of the order is the successive deliveries of stents for the needs of the Employer's Hemodynamics Laboratory.
2. A detailed description of the subject of the contract in terms of the required parameters is specified in Annex No. 2a to the SWZ.
3. A detailed list of the quantities of consumable materials can be found in Annex No. 2 to the SWZ - assortment and price form.
