1. The subject of this order is the purchase of equipment for therapeutic rehabilitation for patients oncology of the Provincial Specialist Hospital of the Blessed Virgin Mary in Częstochowa as part of the multi-annual program entitled National Oncology Strategy in the scope of the task entitled "Purchase of equipment for therapeutic rehabilitation for oncology patients" in 2025, co-financed by Ministry of Health.
2. The detailed scope of the order was specified in Annex No. 2 to the SWZ - assortment and price form and in the Annex No. 3 to SWZ - description of the subject of the order. Other conditions regarding the execution of the order are specified in the draft provisions contracts, constituting Annex No. 7 to the SWZ.
The Ordering Party allows the submission of partial offers. The concept of a partial offer means individual parts specified in Annex No. 2 to the SWZ. The contractor may submit an offer for any parts or for the entire item orders:
1) PART NO. 1 Diagnostic and therapeutic device for upper limb rehabilitation 1 pc.
2) PART NO. 2 Device for automated rehabilitation of hands and fingers of the upper limb 1 pc.
3) PART NO. 3 Kinesitherapy device 1 pc.
4) PART NO. 4 Radiofrequency device 1 pc.
5) PART NO. 5 Deep oscillation device 1 pc.
6) PART NO. 6 Kinesitherapy device 1 pc.
