Description:
1. The subject of the contract is the purchase of medical apparatus (infusion pumps with accessories) along with the maintenance of devices after the warranty period for Copernicus PL Sp. z o.o. The subject of the contract consists of 1 part, which is divided into 6 tasks:
1) Task No. 1 - A set of infusion pumps with stations docking,
2) Task No. 2 - a set of infusion pumps with docking stations,
3) Task No. 3 - Infusion management system,
4) Task No. 4 - Conservation of devices with task 1, 2, 3,
5) Task No. 5 - syringe infusion pump,
6) Task No. 6 - Successive delivery of accessories to pumps from the above tasks.
No. 1a to SWZ - price form,
No. 1b to SWZ - description of the subject of the contract,
No. 3a to SWZ - draft contract for a successive delivery (applies to tasks 1-3, 5),
No. 3b to SWZ - draft contract for a successive delivery (applies to task 6),
No. 3c to SWZ - draft contract for maintenance of devices (applies to task 4).
The Contractor filling out the indicated items in Annex 1B to SWZ-OPZ confirms the fulfillment of the requirements specified by the Ordering Party.
