- The subject of the order is the purchase of medical devices for the General Hospital in Wysokie Mazowieckie.

1. The Ordering Party allows the submission of partial offers (for a package).
2. Description of the subject of the order, size and scope:
The subject of the order consists of the following packages:
Package No. 1: Neonatal respirator.
Package No. 2: Medical equipment for the provision of services in the scope rehabilitation.
Package No. 3: AED defibrillator.
A detailed description of the subject of the order is included in Annex No. 1 to the SWZ - Product and price form, which is an integral part of the SWZ.
The Ordering Party requires that all devices offered in the Assortment and Price Form - Annex No. 1 to the SWZ were medical devices with the exception of item No. 1a in Package No. 2.
Package No. 1: Neonatal respirator. A detailed description of the subject of the order is included in Appendix No. 1 to the SWZ - Form assortment and pricing, Package No. 1, which is an integral part of SWZ.
The Ordering Party requires that all devices offered in the Assortment and Price Form - Appendix No. 1 to the SWZ are medical devices, except for item No. 1a in Package No. 2.
