- 1. The subject of the contract is: Purchase of medical equipment and apparatus for the needs of the obstetrics and gynecological ward of the SPZOZ of the Provincial Specialist Hospital No. 3 in Rybnik divided into 6 parts (hereinafter: part or package)
2. The detachment description of the subject of the contract is included in attachments:
1) form Price (separately for each package) - Annex 1.1. to SWZ,
2) technical specification (separately for each package)-Annex 1.2.A-1.2.f to SWZ,
3) Conditions for the guarantee of quality and service (for each of the packages) - Annex 1.3. 4) planned provisions of the contract - Annex 2 to SWZ.
3. The order of the contract is divided into 6 separate packages (parts), including:
1. Hospital bed with a lippit cabinet and other equipment - 32 sets
2. Gynecological armchair with equipment - 2 sets
3. A trolley for transporting patients in a lying position - 4 sets
4. Infusion pump with equipment - 6 sets
5. Cardiomonitor with equipment - 2 sets
6. Ultrasound apparatus with equipment - 1 set
4. The company allows the possibility of submitting offers for selected parts, but does not allow the possibility of submitting an offer for selected items in a given package.
5. Names not containing full the scope of the subject of the order in a given package will be rejected.
