Description:
The subject of the order is: purchase of medical equipment and apparatus for the needs of the SPZOZ Ophthalmology Department of the Provincial Specialist Hospital No. 3 in Rybnik. The supply specification is included in Annex 1.1, ie "price form". Other terms of the contract are specified in Annex 2 to specifications of the contract terms ("planned provisions of the contract" regarding public procurement, which will be entered into the content of the contract). The order is divided into 6 packages (parts). Agreements in the scope of resolved packages will be concluded after completing these proceedings.
Detailed Description of the subject of the contract is included in attachments:
1) price forms (separately for each package) - Annex 1.1 to SWZ,
2) technical specifications (separately for each package) - Annex 1.2 A - 1.2 F to SWZ,
3) conditions of quality and service guarantee - Annex 1.3 to SWZ,
4) designed contract provisions - Annex 2 to SWZ.
The task received funding from the budget of the Śląskie Voivodeship.
The contracting authority pursuant to art. 257 of the Act na Financing all or part of the order will not be awarded to him.
