Description:
1. The subject of the contract is the purchase of medical equipment for the clinical oncology department. The subject of the contract was divided into 5 parts/packages, each of which is a separate subject of the contract:
Package No. 1 - Chemotherapy armchair
Package No. 2 - Peristaltic power pumps for supplyCytostatics on road racks
Package No. 3 - chairs for patients
Package No. 4 - Hospital beds with foam mattress, lug -in cabinets and an anti -bedsore mattress
Package No. 5 - drip stands
2. Taxpt description of the subject of the contract can be found in Annex 1a to SWZ.
3and the conditions for the implementation of this order should be in accordance with the Act of April 7, 2022 on medical devices and the Regulation of the European Parliament and of the Council (EU) 2017/745 of 5 April 2017 on medical devices or the Regulation of the European Parliament and the Council (EU)2017/746 of April 5, 2017 on medical devices for in vitro diagnostics (if concerned) and with other applicable right regulations in this respect. The offered devices must have a CE declaration.Polish in healthcare units, in accordance with applicable law (declaration of compliance for the entire device, marking CE, notification or excerpt from a medical device to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocial Products).
