Description:
The subject of the order is the supply of medical equipment for the needs of SPZOZ in Łukow.
A detailed description of the subject of the contract is included in Annex 2 to the specification - a statement of technical and functional parameters.
The order has been divided into packages:
Package No. 1 - Mobile and Stationary Loop induction, treatment lamp, stationary ECG, telemedicine data transmission system, gynecological seat, KTG, laser therapy apparatus, ultrasounds, massage rehabilitation table, electrotherapy and ultrasound apparatus, tapped lamp,
Package No. 2 - ultrasound with head, gynecological ultrasound,
Package No. 3 - tomograph of OCT OKA.
General requirements for the subject of the contract:
The contractor undertakes to provide medical equipment, which:
1) must be factory -new, produced not earlier than in 2025, marked and after installation ready for functioning without any additional purchases and investments,
2) must be unused - i.e. not the subject of exhibitions and presentations, unrebred, free from physical and legal defects,
3) meet the competent requirements set in applicable law regarding the admission of the products and devices in the entities healing,
4) have a declaration of medical device and a certificate issued by a notified compliance unit (CE mark), based on the provisions of the Act of April 7, 2022 on medical devices
